Dear Patients,

Telehealth is available for some types of appointments. Please call the clinic to find out more information.

It is important that you understand your insurance benefits.
Call the phone number on the back of your insurance card to check ahead for coverage details.

The physicians of Iowa Arthritis and Osteoporosis Center will not be providing written certification for Medical Cannabidiol (marijuana) Registration Cards.  We understand that some patients may wish to pursue this option and recommend you discuss this with your primary care provider.

Masking Policy

Masking is currently optional for most patients and visitors to Iowa Arthritis and Osteoporosis Center, except in the following cases:

• Individuals who have had a positive COVID-19 test result or known exposure in the last 10 days must wear a mask and notify a member of our team.
• Individuals experiencing any of the following symptoms must wear a mask:

• Nasal / sinus Congestion
• Runny nose
• Cough
• Fever
• Loss of taste or smell
• Sore Throat

This policy is subject to change dependent upon community viral transmission levels and recommendations by the Iowa Department of Public Health and the Centers for Disease Control.

 


Things to know and do before your appointment:

Come prepared – new patients please arrive 30 minutes prior to your appointment to ensure we have all necessary information before your appointment begins.
  • Be sure to check your insurance and request a referral from your primary care provider if required.
  • Bring to each appointment your health insurance and prescription insurance card, a photo ID, and a complete list of the medications you are taking, including the dose. Be sure to include in your list any vitamins or supplements you take.
  • Tell us if you need an interpreter. We want to be sure you have the support you need to understand the information during your appointment.
  • Bring your insurance card. Several insurance companies require authorization for appointments before you are treated at our center.
  • If your insurance company has changed since you were last seen or your appointment was made, be sure to call our office before your appointment to make certain all the pre-authorization work is completed so you will not experience a delay or need to reschedule.

Pre-Visit Check In

For your convenience we offer pre-visit registration and check in.

  • Provide your email address during the appointment scheduling process.
  • Several days prior to your scheduled visit, you will receive an email inviting you to check in for your appointment.
  • This process will include answering a few questions, signing some forms and you can even pay your balance. All steps that will make your visit to the doctor more efficient.
  • This is secure and can be done on your own time, from the privacy of a smart phone, tablet or computer.

Insurance and Payments

  • Our business staff will file all insurance claims for you.
  • We are network providers for most, but not all, insurances. Please verify network status with your insurance to avoid unexpected costs.
  • Co-pay and deductible is due at the time of service.
  • Uninsured patients are expected to pay in full at the time of service.

Your Rights and Protections Against Surprise Medical Bills:

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
If you have No Insurance or select not to use your insurance,  Iowa Arthritis will make every attempt to provide you with a Good Faith estimate upon your request or prior to your scheduled appointment.

  • You are always welcome to call our office (515-270-7222). Our office hours are 8:00 a.m. to 4:30 p.m., Monday through Friday. We provide a 24-hour answering service if you have an urgent need that requires you speak with a physician after the office is closed.
  • Connect with us through our confidential Iowa Arthritis and Osteoporosis Center patient portal. The portal gives you easy electronic access to our healthcare team.
    — You can request your next appointment.
  • — Keep track of medications and treatments that have been prescribed for you.
  • — View lab results after they have been reviewed and release by your physician.
  • — Send non-emergency secure messages or questions to your physician.
  • If you need to cancel your appointment, please call our office at least 24 hours before your appointment so we can properly plan our day.
  • — Failure to provide minimum 24 hour cancellation notice may result in a delay for rescheduling or the decision by Iowa Arthritis and Osteoporosis Center to decline rescheduling.
